Process used to get Medical Staff Bylaws Approved

Can anyone share the process you use to get bylaws approved? Do you have a committee? If so, what does your meeting look like? Is there a specific process you follow (ie. Sending out policies or bylaws in advance, asking for feedback in advance, ETC.)
