In trying to survey Southeast hospitals in our region, we are trying to determine what is required for cardiothoracic surgeons to qualify for TAVR privileges at your facility?

    Please see our criteria below: DEPARTMENT OF CARDIOVASCULAR SCIENCES CARDIOTHORACIC SURGERY/CARDIOLOGY CATEGORY IV CREDENTIALING CRITERIA FOR TRANSAORTIC VALVE REPLACEMENT (TAVR) – Vidant Medical Center (VMC), Greenville, NC has put forth the following credentialing criteria for the center and physician to be qualified to perform TAVR. These criteria are derived from The Centers for Medicare & Medicaid Services (CMS) and statements of Society of Thoracic Surgeons (STS), American College of Cardiology (ACC) and Society of Coronary Angiography and Interventions (SCAI) etc. Initial Requirements: Proctorship: A. Must have approval to be proctored at Vidant Medical Center by an experienced physician for a minimum of two (2) Transaortic Valve Replacement (TAVR) procedures. B. Must have documentation from the proctor reviewing the requesting physician’s cases recommending the Transaortic Valve Replacement (TAVR) privilege without further proctorship. OR: The applicant must provide documentation of equivalent training and experience from another hospital or health care system deemed acceptable by the Chief of Cardiovascular Sciences. Surgeon: A. Board Certified/Eligible in Cardiovascular Surgery. B. Professional experience with: 1. 100 Aortic Valve Replacements (AVR) during career (25 AVR/ year or 50 AVR/ 2 years) including 10 high risk patients 2. 20 AVR in the last year prior to TAVR Cardiologist: A. New operators must be Board Certified/Eligible in either Interventional Cardiology or Pediatric Cardiology. B. Existing operators must be Board Certified/Eligible in either Pediatric Cardiology or Adult Cardiology. C. Professional experience with: a. 50 structural heart disease procedures (lifetime), 5 of which must be TAVR Reappointment: A. Participation in ongoing TAVR programs either randomized controlled trial (RCT) or post-approval study (PAS). B. Maintenance of Board Certifications. C. Must document at least 5 TAVR procedures a year. Proctor Privileges: A. Must meet requirements for medical staff privileges in the Department of Cardiovascular Sciences. B. Must have documentation of having successfully completed at least eight (8) Transaortic Valve Replacement (TAVR) procedures with acceptable outcomes within the last 6 months.

    ____ Transcatheter Aortic Valve Replacement (TAVR) CREDENTIALING GUIDELINES: Current privileges in cardiovascular surgery; and documentation of training in the devices that will be used. (NOTE: as additional devices are added, verification of device training is to be provided to Medical Staff Services or designee). If Structural Heart training/fellowship is greater than 2 years ago, must provide documentation of performance of at least 20 TAVRs in prior year or 40 in prior 2 years. (NOTE: This procedure is performed by a multispecialty team approach – the cardiac surgeon granted this privilege are to include interventional cardiology and vascular surgeons as part of the multispecialty team when performing this procedure.) REAPPOINTMENT CRITERIA: Performance of a minimum of 40 TAVR procedures in the past 2 years or 20 in the prior year (participation as primary or secondary provider is acceptable).
